- To schedule a clinical consultation, contact Rona Bartelstone Care Management and Home Healthcare. The consultation will take 1 –
1.5 hours, but can vary depending upon the complexity of the situation
and the number of persons participating. Consultations are often
done on the phone to accommodate the needs of long distance families.
- During the consultation we will obtain basic demographic information,
health history, social history and a sense of your current concerns
- We will discuss how to present the service to the care recipient,
so that it is seen as a positive, helpful step and not as a
threat to his/her independence.
- After the consultation, the assigned Care
Manager will make a home visit to the care recipient. The
Care Manager will have all of the information provided during
the consultation. This visit will help us to obtain an objective,
comprehensive, professional assessment of the situation, as described
above.
- The Care Manager will be in contact with the family before
and after the initial home visit. The Care Manager will share
his/her assessment and recommendations for meeting the identified
needs. This will be discussed with the family to arrive at a
consensus for proceeding with the plan.
- The Care Manager can then implement the recommended plan of
care, or act as a guide to the family in their implementation
of the care.
Once services are put in place and stabilized, the Care Manager
can stay involved to monitor the quality of the care and be
available for any changes that occur.
The Care Manager’s services can be terminated at any
time, according to the needs of the family.
- The Care Manager can wear many hats including emotional support,
advocacy, coordinator, facilitator, and communicator. The Care
Manager can provide Peace of Mind to the entire family.
- Typically, the care management process can take 5 – 10
hours to complete the assessment and initiate a plan of care.
After services are stabilized the average client uses between
2 – 5 hours of services per month, depending on the complexity
of the needs.
